OT - Therapy Rehab

Job Details:

• Full-time, 1.0 FTE position
• Schedule is typically Monday through Friday, 8:00 AM to 5:00 PM with occasional evening or weekend availability needed
• Provide occupational therapy evaluations and rehabilitation interventions in a home health setting
• Establish and manage individualized plans of care to meet patient needs in the home environment
• Coordinate care with other disciplines and seek orders for appropriate additional services
• Apply knowledge of occupational therapy evidence-based practice guidelines, Medicare regulations, and Conditions of Participation
• Complete CMS required OASIS scoring and collaborate with Quality team on needed adjustments
• Perform point of care documentation for all direct patient care, including skilled services, adverse events, and relevant communications in the medical record
• Participate in multidisciplinary conferences and communicate appropriately with physicians and care team members
• Oversee services provided by Occupational Therapy Assistants and perform supervisory visits within required timeframes
• Take supervision of CNAs when other disciplines are no longer involved in the care plan
• Manage productivity standards, maintain accurate time logs, and log mileage in accordance with agency policies
• Maximize continuity of care and plan efficient travel routes when preparing schedules
• Participate in Quality Assurance Performance Improvement initiatives
• Maintain occupational therapy licensure requirements including continuing education standards
• BS degree or higher from an accredited Occupational Therapy program required
• Current Montana Occupational Therapy license required
• Valid driver's license and appropriate auto insurance required
• Minimum of 2 years of previous OT experience preferred
