Physical Therapist Home Health Job in Lebanon, PA

WellSpan VNA Home Care is seeking a Physical Therapist for home health services in Lebanon, PA. This is a full-time travel position running 91 days, beginning July 6, 2026.

Role Overview

You will deliver physical therapy services to adult and geriatric patients in their homes, completing approximately 6 visits per day across 5x8 Monday-through-Friday shifts. Your responsibilities include gait training, prosthetic training, post-stroke rehabilitation, orthopedic care, neurology interventions, and instruction on adaptive and assistive devices. You will document all patient encounters in the Epic charting system and maintain OASIS documentation as needed.

Requirements

• Minimum 1 year of physical therapy experience
• Prior travel healthcare experience
• Proficiency with Epic charting
• BLS certification (AHA or ARC only)
• Competency in home health therapy delivery

Schedule & Expectations

Work Monday through Friday, 8 hours per day. You will rotate weekend coverage every 6th weekend and provide holiday coverage as needed. Request time off is preferred to be limited to a maximum of 7 days during the assignment.

Compensation & Support

Weekly pay is $2,925.79. Mileage reimbursement is provided at the current rate when submitted with work hours. You will receive 80 hours of orientation upon arrival and are required to complete 5-6 hours of billable modules before your start date.
