Physical Therapist Job in Worchester, MA

Tide Health Group – Worcester Rehabilitation & Health Care Center is seeking a Physical Therapist for a temporary long-term care assignment. This is a full-time position working day shifts, 5 days per week, 8 hours per day, for a 91-day contract beginning June 22, 2026.

Your Role:

• Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs
• Develop and implement appropriate plans of care aligned with regulatory and clinical practice standards
• Utilize standardized assessments and evidence-based practice to guide clinical interventions
• Administer rehabilitation procedures including manual techniques, ambulation training, therapeutic exercises, modalities, and assistive device selection
• Assess patient progress and modify treatments accordingly, documenting all changes in clinical records per regulatory requirements
• Evaluate treatment outcomes and adjust services based on patient response and clinical indicators
• Provide consultation and counseling to patients, families, caregivers, and other service providers
• Communicate with supervisors and health team members regarding patient progress and treatment plans
• Prepare comprehensive discharge summaries per facility and regulatory requirements
• Maintain accurate and timely electronic documentation and patient records
• Record treatment minutes for all services provided per regulatory standards
• Participate in patient care conferences, utilization review meetings, and rehabilitation team meetings
• Attend required facility and company trainings, in-services, and conferences
• Supervise and support physical therapist assistants, rehabilitation aides, and students
• Adhere to APTA practice standards, state practice acts, and professional code of conduct

Requirements: Massachusetts state physical therapy license required. Minimum 1 year of PT experience (new graduates with Level II clinical experience acceptable). Long-term care or rehabilitation experience required. No on-call or weekend duties.

Compensation: $2,563.20 per week.
