Physical Therapist Job in Santa Maria, CA

Join Dignity Health Home Health & Hospice in Santa Maria as a Physical Therapist. You'll deliver patient-centered care in a home health setting, working with patients across a specific geographical region.

Schedule & Duration

• 5 days per week, 8-hour day shifts
• Primarily Monday through Friday, 8:00 AM to 5:00 PM
• Every 3rd weekend requirement
• Work minor holidays; major holidays only if needed
• 91-day assignment starting April 21, 2026

What You'll Do

• Treat patients with orthopedic conditions, strokes, joint issues, and spinal injuries
• Provide home-based physical therapy services
• Complete OASIS documentation (government-mandated form)
• Work within the Cerner electronic medical record system
• Manage patient care within your assigned geographical region

What We're Looking For

• Minimum one year of experience as a Physical Therapist
• Home health setting experience preferred but not required
• BLS Certification (required)
• Proficiency with OASIS forms

What to Expect

• 1.5 to 2-week floor-specific orientation period
• Business attire with white lab coat; no jeans permitted
• Cancellation policy: up to 4 shifts per 13-week assignment
