Occupational Therapist Job in Ada, OK

Tide Health Group - Ada Care Center is seeking an Occupational Therapist for a temporary position in long-term care. You will work days on a 5x8 schedule with no on-call or weekend requirements.

Your Responsibilities:

• Screen and evaluate patients with cognitive and ADL physical functioning disorders
• Develop appropriate care plans following regulatory and clinical practice standards
• Utilize standardized assessments and evidence-based practice for clinical interventions
• Provide rehabilitative, skilled, and medically necessary treatment interventions
• Administer ADL retraining, therapeutic activities, therapeutic exercises, and modalities
• Use supportive and assistive devices as part of rehabilitation plans
• Assess patient progress and adjust treatments accordingly
• Evaluate treatment outcomes and modify services as needed
• Provide consultation and counseling to patients, families, caregivers, and service providers
• Communicate with supervisors and health team members regarding patient progress
• Provide comprehensive discharge summaries per regulatory requirements
• Document information accurately in the electronic documentation system and record treatment minutes per regulatory requirements
• Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
• Attend required facility trainings, in-services, and conferences
• Obtain necessary DME or supplies per regulatory requirements
• Supervise occupational therapist assistants, rehabilitation aides, and students

Requirements:

• Active Oklahoma OT license
• 1+ years of OT experience
• Long-term care or rehab experience
• New graduates considered with Level II experience
• Must adhere to NBCOT and State practice acts, code of conduct, and ethics

Assignment duration is 91 days with a start date of June 15, 2026. Weekly pay is $2,390.40.
