Job Details:

• Provide coverage for Rutland and Bennington County
• Actively monitor and amend patient visit schedules to maximize productivity and patient needs
• Perform comprehensive OASIS and PT assessments including psychosocial, educational, safety, and risk factor parameters
• Develop individualized plans of care to promote optimal functional independence and safety in accordance with APTA guidelines
• Provide skilled physical therapy treatment including therapeutic techniques and patient/caregiver education
• Coordinate patient care with clinical team members including physicians, nurses, caregivers, and other rehabilitation services
• Supervise and direct clinical activities of PTAs, PT students, and home care aides
• Complete and coordinate required documentation ensuring accurate reflection of care provided and outcomes achieved
• Carry out patient-related health assessments including PT/INR, blood pressure monitoring, and other evaluations
• Perform patient-related procedures including staple/suture removal and wound care
• Complete OASIS time points in accordance with agency, state, and federal guidelines
• Collaborate with quality improvement staff to maximize accuracy of clinical notes and OASIS documents
• Coordinate safe patient discharges and/or transfers and ensure all appropriate paperwork is processed
• Participate in in-service training programs and peer review audits
• Proactively engage in marketing and promotion of rehabilitation services within the community
• Accurately complete a variety of agency, state, and federal forms in a timely manner
