Physical Therapist Job in Sunnyvale, CA

Pathways Home Health & Hospice is seeking a Physical Therapist to deliver skilled home health services across the San Jose area. You will work directly with patients in their homes, restoring function and helping them reach maximum performance within their individual capabilities following disease or injury.

• Evaluate and treat patients using medically prescribed physical therapy programs
• Make initial admission and evaluation visits
• Document observations, clinical findings, skilled interventions, treatments, goals, and discharge plans
• Notify your assigned Clinical Team Manager of OASIS scores within established timelines
• Supervise Physical Therapy Assistants when applicable
• Perform duties consistent with your skills and training

Your service territory includes Milpitas, Saratoga, Monte Sereno, Los Gatos, Campbell, San Jose, Morgan Hill, San Martin, Gilroy, Coyote Point, and Sunnyvale. One year of home health experience is required for this position.

Position Details:

• Schedule: 5 days per week, 8-hour shifts
• Duration: 182 days
• Start Date: June 15, 2026
• Weekly Pay: $2,920
