Physical Therapist Home Health Job in Lakeport, CA

You will provide home health physical therapy services to adult and geriatric patient populations throughout the Lakeport area. This is a direct-hire position with Sutter Health Care at Home Lakeside, offering a stable schedule without call, float, or weekend requirements.

Your Responsibilities:

• Complete 5 weighted visits per day, typically serving 4–5 patients daily
• Conduct patient evaluations, assessments, and reassessments
• Develop and implement home exercise programs with short-term and long-term goal planning
• Provide gait training, therapeutic exercise, and functional mobility interventions
• Educate patients and families on safety, injury prevention, and body mechanics
• Maintain proficiency in OASIS documentation and plan of care development

Your Background:

• Valid California Physical Therapy license required
• Valid California driver's license and auto insurance required
• Minimum 1 year of experience as a Home Health Physical Therapist
• BLS certification preferred (AHA or ARC)
• Clinical experience with orthopedic, neurological, and geriatric conditions, including post-op care, stroke rehabilitation, Parkinson's disease, and dementia
• Skilled in adaptive and assistive devices such as walkers, wheelchairs, and Hoyer lifts
• Experience with Medicare A, Medicare B, and Medicaid documentation preferred
