PT - Outpatient

Job Details:

• Work 16 hours per week as part of interdisciplinary team
• Provide professional/clinical knowledge and skills in patient care provision and management
• May be required to float within the same discipline across local campuses
• Use EPIC electronic medical record system
• Required to work 1-2 of 3 summer holidays (Memorial Day, Independence Day, Labor Day)
• Orientation schedule: Every Monday
• 2 years of previous experience required
• At least 1 travel contract completed required
• BLS (AHA) certification required
• Epic experience required
• Iowa state license required
• Must not have been UnityPoint employee in last 12 months
• Must have taxable address outside of 50 miles from facility
• Skills checklist must be completed within 1 year of submission date
• One supervisory reference required at time of submission
