Job Details:

• Block scheduling type
• Conduct 6-8 home visits per day with a minimum of 6 points per day (may increase by 1 point at manager's discretion)
• 1-2 weekends required per contract
• Treat adult and geriatric patients across a variety of diagnoses including med/surg, cardiac, ortho, hips/knees/shoulders, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trachs, trauma, AKA, BKA, and ADLs
• Skilled home care visits under Medicare, Medicare Advantage, and commercial insurance
• Self-schedule visits and hours based on the needs of the HomeCare office
• Use provided laptop, MiFi for internet access, and portable printer for documentation
• Document using BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation system
• Must have valid driver's license and personal vehicle to conduct home visits
• Must have home health experience; OASIS training preferred
• Community hospital and LTAC experience preferred
• 1 year of experience required
• Charting system experience required; Cerner experience preferred
• Required certifications: BLS
• Preferred certifications: LVST, OASIS Trained
• Dress code: Any solid color scrubs
• Active Florida or Compact license required
