RN - Case Management

Job Details:

• Work Monday through Friday from 8:00 AM to 4:40 PM
• No weekend or on-call responsibilities
• Must be a certified care manager
• BSN required
• Minimum 3 years of inpatient clinical experience required
• Minimum 1 year of care management experience required
• Oversee a daily caseload of patients on inpatient units with a variety of care needs
• Attend interdisciplinary rounds
• Complete initial patient assessments
• Facilitate the development of a plan of care in collaboration with patients, families, caregivers, and healthcare teams
• Communicate consistently with medical teams and patients regarding progress of care needs related to post-discharge planning, including home health and inpatient rehab
• Interact with post-acute care referral sources to establish a plan
• Document and place orders for home health, DME, and nutritional needs within EPIC EMR
