Join Kona Community Hospital as an RN Case Manager on the day shift. You will manage acute care caseloads for medical patients, coordinating care and ensuring smooth patient transitions throughout their hospital stay.

Role Responsibilities:

• Manage acute care caseload for medical patients
• Provide case management services in an acute care setting
• Coordinate patient care and treatment plans
• Facilitate communication between clinical teams and patients
• Support discharge planning and post-acute care arrangements

What We're Looking For:

• Valid RN license
• Minimum 2 years of case management experience
• Strong organizational and communication skills
• Ability to manage multiple patients and priorities
