Job Details:

• Monitor and amend patient visit schedules to maximize productivity and meet patient needs
• Perform comprehensive OASIS and PT assessments of assigned patients including psychosocial, educational, safety and risk factor parameters
• Develop individualized plans of care to promote optimal functional independence and safety with patient-specific goals
• Provide skilled physical therapy treatment including therapeutic techniques and patient/caregiver education
• Coordinate patient care with clinical team members including physicians, nurses, caregivers, and other rehabilitation services
• Supervise and direct clinical activities of PTAs, PT students, and home care aides
• Complete required documentation and paperwork in accordance with federal and state regulations and APTA guidelines
• Execute accurate charging and billing processes and address insurance coverage inconsistencies
• Participate in in-service training programs for rehabilitation, medical, nursing staff, and community groups
• Engage in marketing and promotion of rehabilitation services including specialized service development
• Perform patient-related health assessments including PT/INR and blood pressure monitoring
• Carry out patient procedures including staple/suture removal and wound care
• Complete OASIS time points in accordance with agency, state, and federal guidelines
• Work collaboratively with QI staff to maximize accuracy of clinical notes and documentation
• Follow agency protocol with McKesson processes including visit completion and follow-up notes
• Complete various agency, state, and federal forms including HHABN and Medicare notices
• Coordinate safe patient discharges and transfers with appropriate documentation
• Participate in regular peer review audits and provide constructive feedback
