PT - Outpatient

Job Details:

• Perform physical therapy procedures and related tasks under supervision of a physical therapist
• Carry out treatment plans as directed by supervising physical therapist
• Provide variety of treatment approaches using manual techniques and available equipment/modalities
• Modify treatments based on patient progress and communicate status changes to supervising PT
• Document patient progress according to approved medical documentation procedures
• Provide individualized patient and caregiver education
• Assist facility staff as needed to maintain clean, orderly, and effective rehabilitation environment
• Function independently within scope of PTA Practice Act
• Maintain competency in all areas of assigned client care and seek continuing education opportunities
• No weekend requirements
• No on-call responsibilities
• 2+ years of PTA experience required
• Must live outside 50-mile radius of facility
• Required certifications: BLS (AHA)
• Active Illinois Physical Therapist Assistant License required
• Skills checklist current within 1 year required
• Two supervisory reference forms required
