PT - Physical Therapist

Job Details:

• Minimum 1 year of experience required (first-time travelers accepted)
• Home health physical therapy setting covering 4 counties
• Patient population: Adults to Geriatrics
• Patient ratio: 1:5
• BLS certification required
• Must be willing to travel throughout service area
• Charting system: HomeCare HomeBase (HCHB) required
• Any color scrubs acceptable
• Maximum 7 days RTO per contract
• No weekend coverage required
• Floating within facility/scope of practice as needed
• Holiday requirements: Work 1-2 major holidays if they fall during assignment period
• Major holidays include: Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday, Christmas Eve/Christmas Day, New Year's Eve/New Year's Day
• 80 hours of orientation required
• Full compliance required to start on time
• Clinical screening required to determine clinical fit
• Flu vaccine required (medical/religious exemptions only)
