Physical Therapist Job in Antigo, WI

Aspirus at Home - Antigo is seeking a Physical Therapist for a 13-week contract position. You will work Monday through Friday, 8:00 AM to 5:30 PM, with 37 guaranteed hours per week and the flexibility to reach up to 40 hours. This role serves adult and geriatric patients in a home health setting.

Your Responsibilities:

• Evaluate, treat, and plan patient care according to Physical Therapy standards
• Serve patients with orthopedic, neurological, and functional mobility diagnoses
• Document care using NetSmart EMR via laptop for point-of-service documentation
• Meet regulatory and reimbursement requirements through comprehensive documentation
• Average 4.5 visits per day
• Travel occasionally to other branches as needed

Schedule & Logistics:

• Generally no weekends; may rotate one Saturday every 6–8 weeks with a weekday off in exchange
• No holiday requirements
• Cell phone provided for GPS and communication
• Mileage reimbursed at government-approved rate
• Must reside at least 50 miles away from the facility

Requirements:

• Master's Degree in Physical Therapy
• Wisconsin PT License
• Valid NPI number
• Current BLS certification (AHA or ARC)
• Minimum 2 years of home health experience highly preferred

Compensation & Details:

• Weekly pay: $3,124
• Contract eligible for 36 hours of unbillable low census hours
• May be placed on call for low census hours
• Contract duration: 91 days, starting May 18, 2026
