Job Details:

• Assess, plan, treat, evaluate, and implement a plan of care for all assigned patients
• Provide occupational therapy interventions to assist patients in achieving independence with activities of daily living
• Work in collaboration with physicians and other healthcare team members
• Provide supportive supervision of OTAs and OT/OTA students
• Complete documentation of all patient care activities in a timely manner using electronic medical records
• Promote a clean and safe environment of care utilizing SAFE error prevention habits
• Apply evidence-based interventions and uphold professional standards in accordance with AOTA standards of care and code of ethics
• Maintain highest standard of privacy and confidentiality in all patient and coworker matters
• Ability to transfer and position patients required
• Saturday and Sunday rotation work included as applicable
• No on-call responsibilities
• 1 year of Home Health OT experience required
• OASIS and start of care experience strongly preferred
• Valid driver's license and car insurance required
• Required certification: BLS (AHA)
• Illinois state license required and must be active at time of submission
• Skills checklist completed within one year of application required
• Resume and 2 supervisory references required
