PT - Physical Therapist

Job Details:

• Work Monday through Friday from 8:00 AM to 5:30 PM
• 13-week contract with 37 guaranteed hours per week, flexible up to 40 hours
• Occasional travel to other branches as needed
• Generally no weekends, but may rotate one Saturday every 6-8 weeks (given a weekday off when working Saturday)
• No holiday requirements
• Average productivity expectation of 4.5 visits per day
• Evaluate, treat, and plan patient care in accordance with Physical Therapy standards
• Serve adult and geriatric population with orthopedic, neurological, and functional mobility diagnoses
• Document care to meet regulatory and reimbursement requirements
• Use laptop for point of service documentation with NetSmart EMR system
• Cell phone provided for GPS and communication
• Mileage reimbursed at government approved rate
• Must reside at least 50 miles away from facility
• Required certifications: BLS (AHA or ARC), Wisconsin PT License, NPI number
• Required education: Master's Degree in Physical Therapy
• Minimum 2 years of home health experience highly preferred
• Contract eligible for 36 hours of unbillable low census hours
• May be placed on call for low census hours
