Job Details:

• No on-call responsibilities
• No weekend requirements
• Productivity expectation of 25 points per week
• Plan and conduct treatment programs designed to restore and improve physical, social, and mental functions
• Serve as co-leader in interdisciplinary approach to patient groups
• Complete evaluations to ensure quality standards and address assessment areas that require attention
• Use appropriate clinical techniques and tools to establish individualized treatment plans for patients
• Provide therapy interventions using appropriate treatment protocols and techniques
• Establish individualized care and discharge plans and ensure plans are communicated to appropriate parties
• Complete comprehensive and accurate clinical documentation within established deadlines
• Supervise assigned assistants
• Must have own transportation, valid driver's license, and current auto insurance
• Mileage reimbursed at $0.43 per mile
• 2 years of physical therapy experience required
• 1 year of home health PT experience required
• Hawaii state PT license required
• Epic experience required
• Bachelor's degree required; Master's degree preferred
• Travel experience preferred
• Required certifications: CPR, BLS (AHA)
