OT - Therapy Rehab

Job Details:

• Perform 6-8 home skilled visits per day with minimum of 6 points per day
• Self-scheduled visits and hours based on HomeCare office needs
• Work with adult patients including med/surg, cardiac, ortho, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, neuro, trauma, and ADL patients
• Provide skilled home care services for Medicare, Medicare Advantage and Commercial insurance patients
• Must have 1 year experience required, 2 years preferred as independent practitioner
• Home health experience required, OASIS training preferred
• Required certifications: BLS, state licensed
• Must have valid driver's license and car for home visits
• Any solid color scrubs required
• Equipment provided: laptop, MiFi for internet access, portable printers
• Use BayCare Forms, BayCare Scheduling, Ace Timesheet documentation system
• Required skills include: Activities of daily living, cardiac rehab, edema massage, feeding techniques, splinting, TENS, ultrasound, ortho, prosthetics/orthotics, standardized assessment, joint mobilization, home exercise program, lifting and transfer, patient/family education, safety and injury prevention, telephone calls, evaluation tools, billing/documentation
• 40 hours of orientation required
• Florida license or compact license required at time of submittal
• Must live at least 100 miles from facility (100-mile radius rule applies)
