Job Details:

• Provide therapy programs for patients of all ages with developmental deficits, physical injury, or illness
• Coordinate physical therapy services and provide direct therapeutic care to assigned clients
• Teach rehabilitation skills to clients and/or caregivers to promote maximum independent functionality and safety
• Provide preventive and rehabilitative physical therapy procedures individualized to each client based on age-specific needs
• Develop and revise individualized plans of care based on assessments, client history, diagnosis, physician orders, and client goals including discharge planning
• Perform and monitor application of treatments using physical agents such as heat, water, and electricity including whirlpool baths, electrical muscle stimulation, and ultrasound equipment
• Document all skilled and medically necessary care thoroughly, timely, and accurately in compliance with Medicare, State, Federal, JCAHO, insurance, and payer requirements
• Communicate significant clinical findings and changes to physicians and other involved personnel
• Conduct PTA and Home Health Aide supervisory visits as applicable and prepare written instructions accordingly
• Supervise Physical Therapy Assistants and Home Health Aides in accordance with Federal and State laws, rules, regulations, and agency policy
• Participate in Quality Management programs and complete all required in-service programs to maintain current skills and standards
• Participate in staff meetings, case conferences, business development meetings, and customer service programs
• Maintain client and organizational confidentiality at all times
