Occupational Therapist Job in St Johnsbury, VT

Northern Counties Health Care, Inc. is seeking an Occupational Therapist to deliver in-home therapy services to home care and hospice patients in the St Johnsbury area. This is a travel-based role where you'll work independently while remaining connected to a collaborative multidisciplinary team.

What You'll Do

• Provide occupational therapy services to patients in their homes, up to 40 hours per week
• Work independently to maximize patient independence and safety within the home environment
• Collaborate with team members to develop and implement individualized care plans
• Travel to multiple patient homes throughout the service area

What You'll Need

• Current Vermont occupational therapy licensure
• Valid driver's license and reliable vehicle for patient home visits
• Preferred experience in home care settings
• Strong independent work skills and ability to function as part of a broader care team
