Physical Therapist Job in Petaluma, CA

You will work as a Physical Therapist at Santa Rosa Post Acute, a skilled nursing facility in Petaluma, CA. This is a 91-day travel assignment with a start date of October 28, 2025. You will earn $3,358 weekly.

In this role, you will:

• Perform thorough evaluations to assess patients' physical abilities and develop personalized treatment plans
• Provide physical therapy interventions to improve strength, mobility, and functional abilities
• Educate patients and their families on therapeutic exercises, safe movement techniques, and home modifications
• Collaborate with interdisciplinary teams including occupational therapists, nurses, and physicians to coordinate care
• Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes
• Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans
• Ensure care plans align with regulatory requirements and best practices in physical therapy

Requirements:

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license
• Experience working in skilled nursing facility or similar healthcare setting preferred
• Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment
• Excellent communication, interpersonal, and problem-solving skills
• Ability to work both independently and collaboratively in team environment
• Compassionate, patient-focused approach to care
