Opportunity:

RN - Case Management

Job Details:

• Work Monday through Friday, 8:00 AM to 4:30 PM
• Coordinate and facilitate care for patient population through effective collaboration and communication with the Interdisciplinary Care Transitions (ICT) team
• Follow patients throughout the continuum of care to ensure optimum utilization of resources, service delivery, and compliance with external review agencies
• Provide ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs
• Enhance quality of patient management and satisfaction to promote continuity of care and cost effectiveness
• Integrate functions of case management, utilization review and management, and discharge planning
• Minimum 2 years of recent RN Case Management experience required, with experience in a LTAC or STACH setting
• Experience with MCG and discharge planning across acute care settings required
• Strong IDT collaboration skills required
• BLS certification through the American Heart Association required
• Texas RN or multi-state RN license required
