Job Details:

• Shifts available: Days 7:00 AM - 3:00 PM or 7:00 AM - 7:00 PM, Evenings 3:00 PM - 11:00 PM, and Nights 11:00 PM - 7:00 AM
• Every other weekend requirement
• May be asked to work holidays
• No on-call or standby requirements
• Float between either floor as needed
• Dress code: Any color scrubs, must look professional; no large earrings or jewelry, no fake nails, hair must be up, no face tattoos
• Work in a Long-Term Care and Skilled Nursing Facility setting with a 100-bed facility across 2 floors
• Patient ratio of 1:9-12
• Common diagnoses include diabetes, COPD, post-stroke, heart disease, fractures, dementia, Alzheimer's, rehab, and chronic conditions with multiple comorbidities
• Job duties include assisting with ADLs, bathing, toileting every 2 hours, peri care, meal assistance and feeding, transporting residents to activities and appointments, bedtime routines, and 1-2 hourly rounding
• Charting via PCC in conjunction with paper charting
• Must have a minimum of 1 year of prior LTC/SNF experience; 2 years preferred
• Required certifications: BLS and active New Hampshire LNA license
• 3-shift orientation with a preceptor provided
• Supervisor available on every shift as a resource
