You will deliver physical therapy services to patients in a skilled nursing and rehabilitation setting. Your role focuses on helping patients recover from injuries, surgeries, and medical conditions through evidence-based treatment and skilled intervention.

Role Responsibilities:

• Provide direct physical therapy services to nursing and rehabilitation patients
• Develop and implement individualized treatment plans based on patient assessment and clinical goals
• Work with patients requiring skilled nursing care and rehabilitation services
• Document patient progress, outcomes, and clinical observations in the medical record
• Collaborate with an interdisciplinary healthcare team including nursing, occupational therapy, and case management
• Maintain current physical therapy license and required certifications
