What You'll Do

• Evaluate and treat patients in their homes, addressing mobility limitations, pain management, and functional independence
• Develop and implement personalized physical therapy treatment plans
• Document patient progress and communicate with the interdisciplinary care team using Epic electronic health records
• Educate patients and caregivers on exercises, safety, and home modifications
