Physical Therapist Job in Monticello, IA

Above & Beyond Home Health and Hospice is seeking a Physical Therapist for a 12-week travel assignment in Monticello, IA. You'll deliver direct patient care in home and hospice settings, working full-time at 40 hours per week on a 5x8 day schedule.

Position Details:

• Assignment duration: 83 days, starting October 5, 2025
• Shift options: 8:00 AM to 4:00 PM or 9:00 AM to 5:00 PM
• Weekend coverage as needed
• Weekly pay: $2,288

Your Role:

You will provide physical therapy services to patients in their home environment, focusing on improving mobility and functional independence. Your responsibilities include developing and implementing individualized treatment plans tailored to each patient's goals and clinical needs, and documenting patient progress and treatment outcomes in accordance with facility protocols.

Working in home health and hospice, you'll have the opportunity to build meaningful relationships with patients and their families while delivering compassionate, patient-centered care. Your clinical judgment and adaptability will be essential as you work in diverse home settings.

Qualifications:

• Current degree in Physical Therapy
• Epic experience preferred but not required
