Opportunity:

PT - Physical Therapist

Job Details:

• Provide skilled home care visits with 6-8 visits per day (minimum of 6 points per day)
• Work 1-2 weekends per contract
• Must have 1 year of physical therapy experience
• Home health experience required; OASIS training preferred
• Required certifications: BLS and valid state license
• Must wear any solid color scrub
• Valid driver's license and personal vehicle required for home visits
• Equipment provided: laptop, MiFi for internet access, and portable printers
• Documentation systems: BayCare Forms, BayCare Scheduling, Ace Timesheet
• 40 hours of orientation required
• Treat patients with diagnoses including med/surg, cardiac, ortho, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, respiratory conditions, neurology, trauma, and amputations
• Work with various equipment including gait belts, walkers, sit-to-stand lifts, sliding boards, collars, supplemental oxygen, prosthetics, and wheelchairs
• Provide patient and family education, home exercise programs, and safety training
• Must complete pre-start modules and pass EKG exam within 3 attempts
• Must live at least 100 miles from facility location
