Physical Therapist Job in San Ramon, CA

You will provide physical therapy services in a home health setting across assigned county territories in the San Francisco Bay Area. Your coverage areas include Area #12 (San Jose) and Area #11 (Santa Clara, Sunnyvale, Cupertino, Milpitas, Mt. View, Campbell, Los Altos, Saratoga, and Los Gatos). Additional travel outside your assigned area may be required, and weekend coverage may be needed.

Compensation Structure

You will be paid on a per-visit basis using a point system conversion. OASIS visits (recertification, resumption of care, routine, evaluation, discharge, and similar visits) equal 1 point, which converts to 1.33 hours. OASIS Start of Care visits equal 1.5 points, which converts to 2 hours. Your weekly pay is $2,628.

Guarantee Options

• Guaranteed option: 36 hours guaranteed with a minimum of 27 points per week productivity requirement
• Non-guaranteed option: No guaranteed hours; you can refuse patients

What's Included in Your Rate

Your compensation covers drive and travel time, visit time, documentation time, and phone calls to patients and doctors.

Patient Population

You must be willing to see COVID-positive and COVID-PUI (person under investigation) patients.

Required Credentials and Compliance

• Current California PT License
• BLS certification
• Valid Driver's License
• Auto Insurance and Auto Registration
• TB screening (annual)
• MMR, Varicella, Hepatitis B, and TDap (10 years) vaccinations
• Flu vaccine (annual)
• Drug screen and physical examination

Assignment Details

This position begins May 4, 2026, and runs for 91 days. You will work day shift. All requested time off must be disclosed at the time of submission.
