<p><strong>Allied Position Job Notes:</strong></p> <p>Travel or Local Allied Healthcare Professional needed for ASAP start. A minimum of 2+ years of recent hospital bedside experience is required. Candidates must be able to start within 5 weeks. Opportunity for extension upon successful completion of the initial assignment. $750 referral bonus available</p> <p><strong>Opportunity:</strong></p><p>PT - Physical Therapist</p><p><strong>Job Details:</strong></p><p>• Provide physical therapy services in home health setting across assigned county territories<br>• Coverage areas include Area #12 (San Jose) and Area #11 (Santa Clara, Sunnyvale, Cupertino, Milpitas, Mt. View, Campbell, Los Altos, Saratoga, Los Gatos)<br>• Additional travel outside assigned area may be required<br>• Weekend coverage may be required<br>• Must be willing to see Covid+ and Covid PUI patients<br>• Payment made on per visit basis with point system conversion<br>• OASIS visits (ReCertification, RoC, Routine, Eval, Discharge, etc.) = 1 point = 1.33 hours<br>• OASIS Start of Care = 1.5 points = 2 hours<br>• Guarantee option: 36 hours guaranteed with minimum 27 points per week productivity requirement<br>• Non-guarantee option: No guaranteed hours, can refuse patients<br>• Required credentials: Current CA PT License, BLS, Driver&#39;s License, Auto Insurance, Auto Registration, TB (annual), MMR, Varicella, HepB, TDap (10 years), Flu (annual), Drug Screen, Physical<br>• All requested time off must be disclosed at time of submission<br>• Rate covers drive/travel time, visit time, documentation time, and phone calls to patients and doctors</p><p><strong>Facility:</strong></p><p>This healthcare organization specializes in comprehensive home health services, providing skilled nursing and therapy services directly to patients in their homes.
