Physical Therapist Job in Moore, OK

Tide Health Group - Thunder Care and Rehab is seeking a Physical Therapist for a 91-day travel assignment in Moore, OK. This is a full-time position working 5x8 hour day shifts with no on-call or weekend requirements.

About the Role

You will screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs. Your responsibilities include:

• Develop and implement appropriate plans of care following all regulatory and clinical practice standards
• Utilize standardized assessments and evidence-based practice to guide clinical interventions
• Administer rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, and supportive/assistive devices
• Assess patient progress and adjust treatments accordingly, documenting in clinical records per regulatory requirements
• Evaluate treatment outcomes for effectiveness and modify services based on patient response
• Provide consultation and counseling to patients, families, caregivers, and service providers
• Communicate effectively with supervisors and health team members regarding patient progress and treatment plans
• Provide comprehensive discharge summaries meeting regulatory and clinical standards
• Maintain accurate documentation in the electronic system and patient records
• Record treatment minutes for services provided per regulatory requirements
• Participate in patient care conferences, utilization review meetings, and rehabilitation meetings
• Supervise and support physical therapist assistants, rehabilitation aides, and students
• Attend required facility and company trainings, in-services, and conferences

Requirements

• Oklahoma state physical therapy license required
• Minimum 1 year of physical therapy experience (new graduates acceptable with Level II experience)
• Long-term care or rehabilitation experience required
• Knowledge of APTA and State Required practice acts, code of conduct, and ethics

Assignment Details

Start date: June 15, 2026. Weekly pay: $2,390.40. This long-term care assignment offers stability with day shifts and no weekend or on-call obligations.
