Physical Therapist Job in Rutland, VT

VNA Hospice of the Southwest Region is seeking a Physical Therapist for a 91-day travel assignment in Rutland, VT. You'll work 5 days per week on day shifts, earning $2,774 weekly.

Your responsibilities will include:

• Performing comprehensive OASIS and PT assessments, evaluating psychosocial, educational, safety, and risk factors
• Developing individualized plans of care to promote optimal functional independence and safety
• Providing skilled physical therapy treatment with therapeutic techniques and patient/caregiver education
• Coordinating care with physicians, nurses, caregivers, and other rehabilitation services
• Supervising and directing PTAs, PT students, and home care aides
• Completing documentation according to federal, state, and APTA guidelines
• Managing patient visit schedules to maximize productivity and meet patient needs
• Performing patient health assessments including PT/INR and blood pressure monitoring
• Carrying out patient procedures including staple/suture removal and wound care
• Completing OASIS time points per agency, state, and federal guidelines
• Collaborating with QI staff to ensure accurate clinical documentation
• Following agency protocols including McKesson visit completion and follow-up notes
• Participating in in-service training for rehabilitation, medical, and nursing staff
• Assessing patient needs for home care aide services with ongoing supervision
• Coordinating safe patient discharges and transfers
• Participating in peer review audits and providing constructive feedback

This position is based in a hospice home care setting. Your assignment begins March 23, 2026. ADEX Healthcare Staffing handles all credentialing and placement support.
