Physical Therapist Job in West Frankfort, IL

Logan Primary Care is seeking a Physical Therapist for an outpatient position in West Frankfort, IL. This is a 91-day travel assignment beginning April 20, 2026, offering you the opportunity to deliver direct patient care in a community-based setting.

Your Role

You will evaluate and diagnose patients with musculoskeletal, neurological, and orthopedic conditions, developing and implementing comprehensive treatment plans. Your work will focus on providing evidence-based therapeutic interventions to improve patients' physical function and quality of life. You will educate patients and families on exercises and home recovery techniques, and document all interactions accurately in accordance with facility and regulatory standards.

Key Responsibilities

• Develop and execute individualized treatment plans for diverse patient populations
• Collaborate with physicians, nurses, and other therapists to ensure continuity of care
• Maintain accurate and timely documentation of all patient interactions
• Create a safe, clean, and positive clinical environment
• Float to clinically appropriate units as needed within your scope of practice and demonstrated competencies

Required Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
• Active Physical Therapy license in Illinois or compact eligibility
• Current BLS certification
• Minimum of 1 year of recent outpatient experience preferred

Compensation
