Case Management RN Job in Pittsfield, MA

Join Berkshire Medical Center as a Registered Nurse Case Manager on the day shift. This is a temporary position ideal for travel nurses seeking a structured role focused on patient advocacy and care coordination.

Position Details

• Schedule: Five 8-hour shifts per week, days only—no 10 or 12-hour shifts available
• No on-call or weekend coverage required; holiday coverage may apply if scheduled
• Duration: 91 days, starting November 16, 2026
• Weekly pay: $2,610

Your Responsibilities

• Manage healthcare needs and coordinate quality and cost outcomes across the continuum of care
• Review patients at admission and throughout their stay for appropriate level of care and resource utilization
• Perform comprehensive admission and discharge risk assessments related to quality, financial, and patient satisfaction
• Develop transition care plans to secure appropriate post-hospital arrangements
• Work with multidisciplinary teams to negotiate, procure, and monitor services and resources
• Participate in performance improvement initiatives

Required Qualifications

• Current Massachusetts RN license
• 3+ years clinical experience in acute care (internal medicine, surgery, orthopedics, mother/child, cardiology, or oncology) or rehabilitation, home health, and long-term care settings
• BLS certification (American Heart Association or American Red Cross)
• Current Skills Checklist for RN Case Manager (within one year)
• One supervisory reference from employment within the past two years
• Knowledge of Medicare, Medicaid, and third-party commercial payers
• Strong organizational, interpersonal, and communication skills
• Proficiency with electronic medical records; Meditech Expanse experience highly preferred

Additional Requirements

• Personal vehicle required
• Must reside more than 50 miles from the facility
• CCM or ACM certification preferred
• RN-BC (Board Certification in Case Management) preferred
• Knowledge of Joint Commission, DPH, and HealthGrades preferred
