Job Details:

• Minimum 1 year of experience required; first-time travelers accepted
• Patient population: Geriatrics
• 6 visits per day with an average daily census of 115
• Full-time productivity is approximately 25 units per week
• Home Health Occupational Therapy setting
• HCHB (Homecare Homebase) experience required; Epic experience required after 10/1/2026
• If clinician does not have HCHB experience, an online HCHB course must be completed prior to starting
• Mileage reimbursed at IRS standard rate, tracked through HCHB
• Coverage area includes Washington, Johnson, Jefferson, and Emanuel counties with occasional support in Burke, Screven, Bulloch, and Jenkins counties
• Average daily mileage is 50–100 miles
• No weekend shifts required
• Floating required within facility and scope of practice as needed
• Maximum 7 days of requested time off per contract
• If one or two major holidays fall during assignment, the traveler is expected to work both; if three major holidays fall during assignment, the traveler is expected to work two
• Major holidays include Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday, Christmas Eve/Christmas Day, and New Year's Eve/New Year's Day
• Scrub color: Any color
• Required certifications: BLS
• Key skills include adaptive equipment assessment, cognitive assessment, OASIS, home assessment, plan of care, reassessments, and wheelchair evaluation
• Interdisciplinary support available from Physical Therapy
