Physical Therapist Assistant Job in Ada, OK

Tide Health Group – Ballard Nursing Center is seeking a Physical Therapist Assistant (PTA) for a 91-day travel position in Ada, Oklahoma. This is a long-term care role working day shifts, Monday through Friday, with no on-call or weekend responsibilities.

Position Overview

You will deliver rehabilitative and skilled treatment interventions to patients with physical functioning disorders under the supervision of a licensed Physical Therapist. Your work will focus on evidence-based practice and helping patients regain mobility and independence within a long-term care setting.

Key Responsibilities

• Administer therapeutic procedures including manual techniques, ambulation training, therapeutic exercises, modalities, and assistive device instruction
• Assess patient progress and communicate with your supervising Physical Therapist to adjust treatment plans as needed
• Document treatment sessions accurately in the electronic health record system and maintain compliance with all regulatory requirements
• Report on treatment effectiveness and patient outcomes to the supervising Physical Therapist
• Provide consultation and counseling to patients, families, caregivers, and interdisciplinary team members
• Participate in patient care conferences, utilization review meetings, and rehabilitation planning sessions
• Supervise and support students under Physical Therapist guidance
• Maintain accurate treatment time documentation per regulatory and clinical standards
• Collaborate with supervisor and health team members on patient progress and barriers to care
• Adhere to APTA practice standards, state regulations, and professional ethics

Requirements

• Active Oklahoma PTA license
• Minimum 1 year of PTA experience
• Long-term care or rehabilitation experience required
• Skills checklist completion within 1 year of hire
• 0–1 references at time of submission

Compensation & Schedule

This position offers $1,641.60 per week for a 5x8 day schedule (40 hours). The assignment runs for 91 days, beginning July 20, 2026.
