PTA - Physical Therapist Assistant Rehab

Job Details:

• Provide skilled physical therapy interventions under the direction and supervision of a licensed Physical Therapist in accordance with the established plan of care
• Assist patients in improving mobility, strength, balance, endurance, safety, and functional independence within the home environment
• Monitor patient response to treatment and communicate progress, concerns, and recommendations to the supervising Physical Therapist
• Educate patients, family members, and caregivers regarding home exercise programs, safety strategies, fall prevention, and use of adaptive equipment
• Travel throughout Lewis and Carter Counties to provide patient care in homes and community settings
• Collaborate with the interdisciplinary home health team including nursing, occupational therapy, speech therapy, social work, and home health aides to optimize patient outcomes
• Maintain accurate, timely, and compliant documentation according to agency, state, federal, and payer requirements
• Participate in staff meetings, case conferences, quality improvement initiatives, and continuing education opportunities
• Full-time position with fleet vehicle and paid gas card provided
• Dedicated service area focused on Lewis and Carter Counties
• Graduate of an accredited Physical Therapist Assistant program required
• Current Kentucky PTA license in good standing required
• Current BLS/CPR certification required
• Valid driver's license and proof of automobile insurance required
• Home health experience preferred but not required
• Minimum 1 year of experience required
• Must reside outside of a 75-mile radius (therapy department may be lenient on radius rule)
