Day-to-day responsibilities:

• Complete initial evaluations and progress assessments in patients homes
• Develop individualized treatment plans focused on mobility, strength, and return-to-function goals
• Provide hands-on therapy sessions and educate patients and family caregivers on home exercise programs
• Coordinate care with nursing, occupational therapy, speech, and the referring physician
• Document visits in the agency EMR in accordance with Medicare home health guidelines

What we expect from you:

• At least 2 years of recent clinical Physical Therapy experience (home health preferred)
• Active PT license valid in North Dakota (or willingness to obtain via compact)
• Reliable transportation for home visits across the service area
• Current BLS certification
