Job Details:

• Work Monday through Friday, 8:30 AM to 5:00 PM with no weekend or holiday requirements
• Provide in-home physical therapy services including admissions, existing patient visits, and recertifications
• See 4 to 6 patients per day with the ability to start shift from home without stopping at an office
• Travel within a 50-mile radius east and west, approximately 60 miles north up to Sun Valley, and approximately 40 miles south to the ID/NV border (no mileage reimbursement provided)
• Treat a multidisciplinary caseload consisting primarily of adults with approximately 78% Medicare patients including elderly, rehab, CVA, and inpatient surgical referrals covering orthopedic, surgical, and wound cases (no lymphedema)
• Participate in Monday and Wednesday multidisciplinary meetings to discuss current admissions
• Document using Epic with remote client offshoot
• Must have at least 1 year of post-graduate physical therapy experience
• Must have home health experience; post-graduate home health experience preferred
• Must have orthopedic experience

• Business casual attire or any color scrubs required
• Department-specific orientation includes Epic training classes and up to 1 week of shadowing with another therapist
• Cancellation policy allows up to 4 shifts per 13-week assignment
• Required certification: BLS (AHA)
