RN - Case Management

Job Details:

• Work 5x8 shifts Monday through Friday from 8:30 AM to 5:00 PM, 40 hours per week
• Agency staff may be scheduled up to every other weekend based on staffing needs
• One holiday per year is required, consistent with staff expectations
• No on-call required
• Float coverage will focus on the Emergency Department and medical inpatient units, and may include coverage of more than one inpatient unit based on staffing needs
• Typical assignment is one 20-bed unit in partnership with a Social Worker; during crisis staffing, may support approximately 1.5 to 2 units
• Average daily census of approximately 40 patients
• Support adult and geriatric patients with discharge planning and patient transitions in partnership with Social Work
• Manage medically complex Med-Surg and Critical Care patients including diagnoses such as diabetes, heart failure, COPD, respiratory failure, arrhythmias, pneumonia, pulmonary edema, and pulmonary embolism
• Coordinate discharge needs, resources, and transitions of care
• Minimum 2 years of hospital inpatient RN Case Management and discharge planning experience required
• Med-Surg and/or Critical Care discharge planning experience required
• Therapy-only backgrounds will not be considered
• Must be able to work independently after approximately 3 days of orientation
• One pre-approved time-off request permitted per contract; no RTO approved on holidays or hard-to-fill days such as Easter, Mother's Day, Father's Day, etc.
• Required certification: BLS
• Active Maryland RN license must be in hand upon submission
