PT - Physical Therapist

Job Details:

• Plan and execute rehabilitation programs to help patients return to a higher level of physical function after an accident or surgery
• Help manage or lessen pain experienced by patients with degenerative conditions or terminal diseases
• Work in a primarily outpatient setting (90% outpatient, 10% inpatient)
• Consult with patients to learn about their physical condition and symptoms and review medical history and referrals
• Diagnose movement dysfunction by observing and listening to patients
• Develop individualized treatment plans for each patient
• Coach patients and teach therapeutic exercises and stretches to improve their condition
• Perform hands-on therapy such as massage to ease pain and promote healing
• Collaborate with speech therapists and other care providers to achieve best patient outcomes
• Maintain detailed records relating to patient conditions, care plans, therapies, and progress
• Report patient progress and current status to appropriate parties including family members and providers
• Master's Degree from an accredited physical therapy program required
• Experience working directly with patients in a long-term care or hospital setting required
• Current Physical Therapist state license required
• Current Basic Life Support (BLS) certification required or ability to obtain within 6 months of hire
• Strong documentation skills and excellent verbal communication required
• Demonstrated knowledge and skills to provide care appropriate to the age of patients served
