Job Details:

• Home health occupational therapy position serving a geriatric patient population
• Expected to complete 6 visits per day with an average daily census of 115
• Full time productivity is approximately 25 units per week
• Coverage area includes Washington, Johnson, Jefferson, and Emanuel counties with occasional support in Burke, Screven, Bulloch, and Jenkins counties
• Average daily mileage is 50–100 miles per day; mileage is paid at the IRS standard rate and tracked through HCHB
• No weekend shifts required
• Floating within facility and scope of practice as needed
• Maximum 7 days of requested time off per contract; all RTO must be finalized at time of submission
• Holiday work expectations apply based on number of major holidays falling within assignment period
• Scrub color: any color
• Required certification: BLS
• HomeCare Homebase (HCHB) experience required until 10/1/2026; Epic experience required after 10/1/2026
• If clinician does not have HCHB experience, an online HCHB course must be completed prior to starting
• Minimum 1 year of experience required; first-time travelers accepted
• Key skills include adaptive equipment assessment, cognitive assessment, OASIS, home assessment, plan of care, reassessments, and wheelchair evaluation
