PT - Physical Therapist

Job Details:

• Evaluate, plan, direct, and administer Physical Therapy services as prescribed by licensed providers
• Work in inpatient, outpatient, and/or home health settings
• Treat patients with Physical Therapy modalities including ultrasound, NMES, and iontophoresis
• Perform skilled therapy techniques such as manual muscle testing and joint ROM measurements
• Instruct patients on use of adaptive and assistive equipment
• Supervise and direct Physical Therapy Assistants (PTA)
• Perform appropriate patient assessments related to physician orders
• Design treatment programs for patients with varying degrees and types of disabilities
• Formulate realistic short and long-term treatment goals
• Include patient and family in development of care plans
• Ensure accurate patient charges and documentation in compliance with regulatory standards
• Complete timely and efficient documentation including evaluations, progress notes, monthly notes, and discharge summaries
• Current Illinois Physical Therapist License required
• Valid driver's license and proof of personal auto insurance required
• BLS certification required
• APTA membership preferred
• Minimum 1 year of Physical Therapy experience preferred
• Strong computer skills including Microsoft Word, Excel, and Outlook required
• Ability to work independently and prioritize tasks effectively
