Job Details:

• Provide occupational therapy services in a home health setting
• Monday through Friday schedule, 8:30 AM to 5:00 PM
• 8 hours per day, guaranteed 40 hours per week
• No on-call required, however 1 weekend per month required for visits due to orthopedic cases
• 1 major and 1 minor holiday required
• Average of 4 to 6 patient visits per day
• Coverage area: East Lake County
• Average miles per week varies; IRS mileage reimbursement provided
• 13-week assignment
• Minimum 1 year of occupational therapy experience required
• Florida OT license (OT-FL) required
• SSN and date of birth required for credentialing
• Valid driver's license and auto insurance required
• 1 reference from entire work history required
• National certification required
