PT - Physical Therapist
Job Details:
• Provide general Physical Therapy evaluations and treatments impacting patient functional abilities across a wide range of ages and diagnoses
• Perform assessments, evaluations, and therapy diagnoses while formulating and implementing individualized treatment plans with goals
• Implement, supervise, and modify therapy plans of care based on patient response to optimize outcomes
• Work with both in-patient and out-patient populations in various settings as assigned
• Review clinical records and case histories, attend care conferences, and collaborate with referring providers and healthcare professionals
• Collaborate with team members to ensure appropriate medical equipment is arranged and care plans are established prior to patient discharge
• Direct the work of Physical Therapist Assistants, support staff, students, and interns
• Develop, implement, and conduct patient education and therapy programs based on patient and family needs
• Provide patients and families with clear and concise home exercise programs to achieve fullest rehabilitative potential
• Assist in acquisition and fitting of durable medical equipment and devices based on patient needs
• Maintain cleanliness of clinical space and keep supply stocks and clinical equipment at acceptable levels
• Educate staff on PT-related issues and educate patients and families about functional limitations, therapeutic exercises, and equipment
• Adhere to regulatory and practice standards set by the American Physical Therapy Association (APTA)
• Entry level qualifying master's or doctorate degree in Physical Therapy from an accredited program required, OR graduate from an APTA approved program at the master's or bachelor's level with 2 years of proven work and Continuing Education experience
• Minimum 1 year of experience in adult outpatient rehabilitation required
• Active Alaska Physical Therapist license required at time of submission
• Current Basic Life Support (BLS) certification required
• Valid Driver's license required
• Must live at least 50+ miles away from the facility address
