Job Details:

• 8-hour day shifts, Monday through Friday from 8:00 AM to 5:00 PM
• Home health care setting serving patients across Clark, Edgar, Effingham, Champaign, and Madison County
• Treat approximately 8 patients per day with a productivity expectation of a minimum 6.5 points per hour, typically 3-4 evaluations
• Primarily treat post-acute patients in a home care environment
• Provide supervision to 1 COTA on staff
• Self-scheduling with flexibility to manage and move visits throughout the day
• No weekend or holiday requirements
• Scrub color or business casual attire acceptable
• Must have home health experience and Home Care Home Base experience
• Required certification: BLS (AHA) and CPR
• Charting completed using a computerized system
