Job Details:

• No on-call or weekend requirements
• Minimum 2 years of physical therapy experience in a long-term care (LTC) setting required
• Texas license or compact license required
• Perform patient evaluations by obtaining patient history, conducting systems reviews, and administering tests and measures to collect data
• Establish individualized plans of care and select treatment techniques that address specific impairments identified during evaluation and in accordance with physician orders
• Conduct ongoing reassessment of patients and their response to treatment
• Initiate and manage discharge planning beginning at the first visit and continuing throughout the course of treatment
• Supervise and delegate to ancillary staff and students in accordance with state laws and department policy
• Complete accurate and legible documentation within established timeframes and in accordance with accreditation requirements and insurance regulations
• Communicate with patients, families, caregivers, and members of the healthcare team to promote maximum benefit of care
• Assess and address educational needs of patients, families, and caregivers
• Participate in professional growth and development through educational programs, in-service meetings, workshops, and current literature review
• Actively participate in quality improvement activities
• Maintain a safe environment for patients, visitors, and staff in compliance with approved safety standards
• Function within scope of practice as defined by the state's practice act
