About this role
VNA Hospice of the Southwest Region is seeking a Physical Therapist to provide skilled therapy services across Rutland and Bennington County. You will work a 5x8 day schedule for a 91-day travel assignment beginning June 18, 2026, earning $2,774 per week.
Your Responsibilities:
- Provide comprehensive OASIS and PT assessments, evaluating psychosocial, educational, safety, and risk factors
- Develop individualized plans of care to promote functional independence and safety in accordance with APTA guidelines
- Deliver skilled physical therapy treatment using therapeutic techniques and patient/caregiver education
- Monitor and amend patient visit schedules to maximize productivity while meeting patient needs
- Coordinate care with physicians, nurses, caregivers, and other rehabilitation team members
- Supervise and direct clinical activities of PTAs, PT students, and home care aides
- Complete patient-related health assessments including PT/INR monitoring, blood pressure checks, and other evaluations
- Perform patient procedures such as staple and suture removal and wound care
- Complete OASIS time points in compliance with agency, state, and federal guidelines
- Ensure accurate and timely completion of all required documentation reflecting care provided and outcomes achieved
- Collaborate with quality improvement staff to maximize accuracy of clinical notes and OASIS documents
- Coordinate safe patient discharges and transfers with appropriate paperwork processing
- Participate in in-service training programs and peer review audits
- Engage in community marketing and promotion of rehabilitation services
- Complete agency, state, and federal forms accurately and on time
