Job Details:

• Assess client conditions and develop individualized plans of care to maximize mobility and independence
• Collaborate with interdisciplinary team and colleagues to ensure client needs are met
• Perform standard therapy measurement techniques including ROM, strength, and balance tests
• Utilize a variety of therapeutic modalities to maximize care provided to clients
• Provide personalized home health therapy care to clients in their own homes
• Case manage clients including OASIS documentation
• Vermont PT licensure required
• Home health experience required
• OASIS experience required
• McKesson/Netsmart/Homecare Homebase EMR experience strongly preferred
• No requested time off - quick start position
