Physical Therapist Job in Mishawaka, IN

Trinity Health PACE Saint Joseph PACE is seeking a Physical Therapist to join their interdisciplinary care team in Mishawaka, Indiana. This is a full-time position working days, 5 days per week, 8 hours per shift, for a 152-week assignment beginning August 10, 2026.

About the Role

You will provide physical therapy services within a Program of All-inclusive Care for the Elderly (PACE) setting, working directly with a geriatric patient population. Your responsibilities include:

• Conducting individualized physical therapy assessments and developing treatment plans tailored to patient needs
• Delivering therapy services focused on supporting patient mobility, functional independence, and overall quality of life
• Collaborating with an interdisciplinary care team—including physicians, nurses, social workers, and other specialists—to ensure coordinated, holistic patient outcomes
• Monitoring patient progress and documenting all clinical findings in accordance with facility policies and regulatory standards
• Supporting patients in achieving their functional goals within the PACE model of comprehensive elderly care

Requirements

You must hold a valid degree in Physical Therapy and an appropriate state license to practice in Indiana. Experience working with geriatric populations is beneficial.
